Burneston Dental Care Plan Terms and Conditions

  • It is a condition to acceptance on our dental plan that all patients have good standard of dental health before being accepted onto the plan. New patients shall undergo an initial dental examination with one of our dentists to determine their general dental health. If there are dental treatments required to bring the patients dental health up to the standard required for the plan, the dentist shall inform the patient what is required and the charges for those treatments.
  • The Dental Care Plan is a scheme to provide preventative dental treatment over a twelve-month period/cycle and to provide additional discounts and / or benefits to plan members as stated in the current Burneston Dental Care Plan Leaflet, reflected online
  • Plan members are entitled to the benefits of their plan from the date their application is accepted by Burneston Dental until the plan is terminated by either party.
  • Plan members have a 14-day cooling off period from the date they sign their Dental Plan application form in which to cancel their plan. If a plan member cancels within this 14-day period any contribution, they have paid to the plan will be refunded in full provided they have not already benefited from any of the benefits available under the plan.
  • No refunds will be given after the 14-day cooling off period. Please ensure you make use of the plan and visit us for your allocated appointments within the specified plan year.
  • Burneston Dental will send complimentary reminders for plan appointments, but it is up to the plan member to make sure that he/she benefits from the plans benefits in the yearly cycle. If the member does not attend or take advantage of his/her Burneston Dental care plan appointments/benefits, no credit is due/accumulated.
  • If a plan member cancels within 6 months from the date of signing up and has already benefited from benefits of the plan, it will be up to the discretion of Burneston Dental to assess if any charges apply.
  • Plan members may not be able to book appointments if there is a balance outstanding on the account. This will need to be settled before appointments can be booked.
  • Plan members can request to take a payment holiday. This is to be agreed with Burneston Dental prior to the taking of the payment holiday. The requests will be assessed by Burneston Dental on a case by case basis.
  • By signing up to the dental plan you agree to be bound by these Terms and Conditions; you agree to pay all fees on time (collected by direct debit).
  • Burneston Dental Care plan is a monthly rolling contract.
  • Burneston Dental reserves the right to vary or make changes to the plan, after giving 30 days’ notice of any such proposed changes.
  • Our standard terms and conditions governing failed appointments, cancellations, short notice and fail to attend charges apply.
  • A deposit may be required at the start of any treatment you receive.
  • Full and prompt payment must be made to be eligible to receive the agreed discount.
  • Being a member of the plan entitles you to receive a 10% discount on all of the treatments carried out by your dentist at Burneston Dental. (Some exclusions apply. Please see website or practice leaflet for details).
  • The 10% treatment discount cannot be used in conjunction with any other promotion available in practice.
  • If you default on two successive payments, we will inform you that your plan has been cancelled. Burneston Dental Care Plan cover will cease from the date of the first failed payment.
  • If you fail to reinstate payment of the monthly fees and are removed from the plan, any outstanding treatments will be charged at the full price of the original treatment cost.
  • If you cancel your plan, any outstanding treatment plan that has been estimated utilising the 10% discount benefit will be re-calculated and a new treatment plan will be issued to you detailing the full cost of the treatment.
  • You must cancel your plan in writing giving 30 days’ notice.
  • Monthly fees will be reviewed on an annual basis; we reserve the right to increase these fees giving members at least 30 day’s written notice.
  • Any disputes arising under this agreement shall be settled by arbitration if they cannot be settled through our complaint’s procedure.
  • Burneston Dental is responsible for processing your Direct Debit payments for the dental plan.
  • The information contained in these terms and conditions is subject to change without notice. All new terms will be updated on our website as and when required.
  • These terms and conditions and the policy document shall be governed by and construed in accordance with the laws of England and Wales and will be subject to the exclusive jurisdiction of the English Courts.
  • You may not post, distribute or reproduce any copyrighted material, trademarks, or other proprietary information without obtaining Burneston Dental permission of such rights.
  • All notices under this agreement will be sent by post to the last known address of the plan member.
  • A person who is not a party to this contract has no rights under the Contracts (Right of Third Parties) Act 1999 to enforce any of these terms and conditions or the provisions of the Policy Document.
